- More than anything else, this form can cause huge headaches
during your
last semester and even delay graduation. Please follow all
instructions!
- Due early in your last semester; can be done prior.
Recommendation: complete as soon as you know what your courses will be
all the way to the end.
- This form can be filled in, saved, and emailed.
- Do NOT send it to the Graduate School! It must go to your advisor.
- Not easy! Create a draft and submit to your advisor as
early as possible!
