Survey Template

To use this page, choose to "save as.." a web page (give it any name you want, just make sure it ends in '.htm'). Then change the content and hidden fields to suit your purpose.

This templates uses a web form to ask a survey. When the user presses "submit", the information is emailed to an email address that you specify. All this is made possible by choosing to POST the information from all the form's fields to a special CGI script available on a UGA server. (In Dreamweaver, click somewhere inside the form, as defined by a dotted red line, choose the tag <form>, then look in the Property Inspector to see the URL of the CGI script in the "Action" field.)

The form contains 4 very important hidden fields:

• MailRec - contains the email address you want the form information sent to (currently set to "lrieber@uga.edu"; obviously, you should change this to your email address)
• MailSub - the subject line of the incoming email message
• ThankYou - this contains the word "mine" that tells the CGI script to send the user to a URL you specify that thanks them for their response (if left out, the CGI script sends them to a generic URL).
• ThankYouURL - the URL of the web page that thanks the user for their response (currently set to a simple page that I created; if you use this template, be sure to change this to point to a HTML page that you create).

The results are sent to an email address that you specify in a hidden field. The subject line of the email is also contained in a hidden field. Another hidden field contains the URL of a HTML page that lets the user know they response was sent.

You can add as many questions as you like (using whatever question formats you like). As long as the questions use form fields that are contained in the form shown below.
